Kayo ends up being blackmailed by Iketani to keep having a sexual relationship with him and she pretty much tries to commit suicide, forcing MC to resolve the issue

As for Kayo: she cheated for the same reason Miyuki cheated in Jinsei Gyakuten -- she's spoiled and too used to MC being there that she took him for granted. She wanted to know what it was like to bang a fukboi, liked the feeling, couldn't stop. Kayo still totally loves and intends to spend the rest of her life with MC, but she couldn't resist temptation.

Ah that's unfortunate. I was kinda interested to know if Kayo had other reasons for not breaking up when she clearly enjoyed being with the other dude more. Avoiding physical contact with the MC while claiming to still be in love doesn't make much sense unless she was intentionally making the MC feel jealous but that wasn't the case here.

Kayo was surrounded by slutty bitches all her life. Her basketball members are slutty, cheating bitches that were the ones pushing for Kayo to cheat in the first place. Her mother also cheated on her partner. The difference with her mother is that the partner decided to accept another person's child (Kayo) and marry her mother.

Kayo was addicted to the excitement having lots of sex. And the sheer thrill of cheating behind MC average, boring MC back with fun, exciting, popular, handsome fukboi she shared basketball with.

Kayo does love MC. It's just that addiction makes people stupid. She got carried away with having her fun that she kept blowing off MC until she got pregnant. Being childhood friends that lived next door to each other their entire lives, Kayo got used to MC always being around and always being there for her. It is the exact same thing that happened with Kayo's mother and her stepfather.

While it's true Kayo might have lost some interest with MC over the exciting Iketani, she still loves MC as a provider/comforter. Think of it as the difference between loving family and a sex friend. Kayo does not love Iketani. Kayo loves the excitement of cheating and having sex with Iketani. When Kayo dreams of having children and showing off her husband to her family, MC is the one she thinks of.

The reason why Kayo neglected MC is because she got too carried away, thinking MC will always be there for her.
