Is she back in her body? I'm confused or at least I think I am which is making me more confused. I don't think Sachiko's first thought after waking up in the hospital would be Madoka. Plus whoever is in Sachiko's body didn't seem to know what was going on.

I think it's quite obvious that Madoka (with her flags as red as the Soviets) caused Sachiko to orchestrate the injuries in the first place hence why Sachiko's first thought is of her after waking up.
