i'm sure we'll get some 'tragic backstory/flashback' some time soon lol (although other than other cases existing before, it is interesting how madoka 'knew' b/c normally it's impossible to swap bodies with someone versus like "i got in an accident and it affected my memory/personality")

I think it's been made pretty clear Madoka loves Sachiko as a kind of doll. The added subtext there is that she wants someone pliable and silent as a partner which is how Yuzu/Sachiko got rejected for being too lovey dovey. I'd wager swapping bodies has meant Sachiko/Yuzu has more of a presence to reject Madoka. So Madoka, in essence, has rejected Sachiko bodily and been rejected by her personality. She's lost Sachiko literally body and soul, which has sent her spiralling and I bet that Yoh is now her new replacement Sachiko.
