The girl carried away his noose. So unless it's some really sophisticated hallucination where he removed the noose and hallucinated the girl doing it for him (which implies he predicted people would come check his car), this girl is as real as flesh and blood.

Best explanation is the girl probably wanted to do away with the noose so he would remain alive long enough to take her away from her own problems.

ok so the diary says he can't sleep and he also said that before. so likely he sometimes just completely shuts down after not sleeping for a long period of time, explaining how the girl said he was just staring at her blankly the first time and him having no recollection of such thing. and maybe when he's in that shutdown mode, his body autopilots or another personality surfaces

I wouldn’t be surprises if author-san went down the schizo route. Doesn’t explain the rope save but mangaka aren’t that finicky so I wouldn’t rule out the bratty kid being a ghost/a hallucination.

I didn't even think of that. But reading this made me think of this...he says that he is indecisive. What if that girl was asking for help from him or he saw signs of abuse but didn't do anything because of indecisiveness and she ended up dead. Now he "brought her along" because of his guilt for not saving her.
