Welp, now it's really confirmed she's an entity of some sort and is no longer human

Her smile and laugh though. It was sooo creepy even after it transitioned to her child look.

so she's a ghost/youkai because she wqs able to interact with the bartender, imma take a wild guess and say the fat manager or parents maybe killed her since in the ending it shows portfolios and it says she often visited that mcs work place

I guess if they reveal that Chiaki is a malevolent ghost so early on, it might leave room for Ryouhei to do something about his situation.

Okay, this answered one question and created several more. So it's clear now that the girl is dead and is now a ghost attached to him, but there were some hints that definitely stuck out:
First, how can Yasu see her? Two possibilities, either Yasu herself is dead too or Chiaki let herself be seen.
Second, was Chiaki killed by MC? It's still to soon to tell but I believe that's not the case, according to the profiles at the end, Chiaki is 11 years old and her ghost self is 35, meaning she has been dead for 24 years, that'd mean MC was 7 years old when she died. There are other proofs supporting this, such as Chiaki referring Yasu as "Yasu-chan", an honorific used only for younger people; the mysterious woman calling her "Chiaki-onesan" ergo Older sister Chiaki; and that her memorial is in the highway, implying she died in an car accident.
Third, why is she attached to MC? We don't know yet, but we know it wasn't accidental, because Chiaki mentioned "she choose him" and that he will "take her to his finish line", so she definitely has a plan of sorts for him.

I wonder if she stuck to him due to his parallels with the originator of the Lake. They were both swallowed by anger and left a mess in their wake involving the dead.

The extra page called her a 亡霊 bourei which is an apparition ghost based on someone who had died, so at least she's not a 幽霊 yuurei malevolent spirit who's just there to stir things up and bring suffering.

The parallels between him and the originator is kind of creepy though. Could she be leading him down the same path? Making him wallow in regret as he offs himself? Or, as she put it, bringing himself to his own finish line? He could've ended it all simply with the suicide he originally planned, but nope, she convinced him to prolong the journey to the same ending.

Presumably the car was the first connection. She died in a car crash, the MC bought the used car around the same time he encountered her, and she's made comments about the car being familiar.
This makes a lot of sense since the, idk memorial items, were out on the street

Some people are so sensitive to seeing lolicon content that they would bolt at the merest suggestion to it when the book cover is already unlike common lolicon stories.
Thank god the author for using the normie repellent.
