If she likes gothic lolita style, I can easily imagine her liking scars as well. Gothic lolita styled characters are often given stuffed animals with bandages and wounds.

It's still possible it is from a fight, but he wouldn't admit it just like that. There are all kinds of accidents as well, some more dubious than others.
