Background:
Shirikodama is almost certainly a reference to the prostate.

After drowning (or nearly any death where you slowly lose oxygen like strangulation) your muscles relax before rigor mortis. That is, you will piss and shit your pants (if you are wearing them) so when your body is found you will usually still have poop in them. If you drown in slow water, your pants will be full of shit. Thus the Kappa myth developed to them attacking you via the butthole.

If you do a hand inspection of a corps, the prostate will be atrophied. that is, the little bump will be missing. Thus the myth of the Shirikodama and the Kappa reaching up your bum and stealing your "life pearl".
