Last time I've read a manga with a character failing their exams and taking supplementary lessons was with henneko. Usually, there be a studying session with the cast and all of them pass the exams therefore not taking supplementary classes just to enjoy summer. Actually refreshing to not see it once in a while.

From all the manga/anime, and real world sources, I'm off the opinion it's not super easy to fail classes in Japan, but it happens occasionally for the story's sake. But yeah, one should fail classes in high school if they don't study at all.
