There's a mistranslation here:
Neither she, nor he calls the bully "Kai-kun". He calls him "Gai-kun" (his given name) and she calls him "Arai-kun" (his surname). It's big because if you know how in Japanese the names you use relate to interpersonal relations, you might mistake her being closer to Gai than she really is.

"kato from the kami" should be "Ice God's Cage" (hyoujin no kago)
