Sorry late response. Its more of the husbands crippling self confidence, where he's constantly questioning why someone like her married him and "maybe she could be better off with someone better then me".
She on the other hand is totally in love with him. She only requested a separate bed because if they slept together, she would basically bang him every night.
However, all these things mostly go unsaid, and the characters are left to their own wild imaginations. No NTR, just self confidence issues and a lack of communication. Typical romance manga stuff from my stand point
