People wondering why nobody noticed--It all happened in a few seconds. Probably people are noticing, but if they're not Ijin it will take them some time to realize the significance of what they're hearing, decide what to do, start heading for the trouble, and actually get there.
And for that matter, some people will be thinking "That sounds like a violent fight, maybe deadly. I'm a nurse, they don't pay me for this shit, think I'm going to go the OTHER way. Lessee, what was the number for security again?"

I dunno why Asian dramas with gangsters in them seem to think that hospitals have no staff around whatsoever during the night. I could understand if some were bribed but there's no way a cop wouldn't have another cop guarding them. I would at least think that it would be standard procedure after a deadly attack on one of their own.
