IIRC, plastic surgery can cost around 300,000-1.5 million yen (roughly $2,000-10,000 USD), so it's possible Mikoto doesn't want to burden her mom with the surgery costs.

Also to add to this about the cost there really is only so much plastic surgery can do, look up Katie Piper, she was an actual model so she could afford the best medical care and even so she still has quite bad scarring
