The dream was caused by the sleeping girl, shes got the same hairstyle, outfit and tail, and she was mumbling about where her feast is. Likely some kind of dreameater.

sleeping girl is very likely a Baku, a mythological creature said to have a trunk, and eat dreams/nightmares (Munna and Musharna are Pokémon based on Baku). Akigai's dream is prob based off of his (as of now unknown to us and likely forgotten to him) past with Mizuda when he was a kid, sleeping girl just prob invaded intending to devour it without anyone knowing, and Mizuda interfered to calm him down, ended up appearing physically in the dream as well since you know, she's pretty obviously some kind of Lovecraftian based creature

Most likely, but then again Mizuda keeps getting represented in the same outfit over and over again, so it's possible it was mixed with some truths and not sure I'm misremembering, but i recall him wearing a eyepatch as child in one of the earlier chapters, though we were never shown how it came to be how he even supposedly met Mizuda as child

So could've been an actual dream, but that girl could've snuck in there
