I think people don't pay much attention to details just because it's a 4-koma (since they are supposed to be relaxing and stuff), the author has already given a lot of hints in the last few chapters, like, the dog ear girl likes to eat a lot and will eat anything (probably because her mother died of hunger), the red ribbon girl was created by Raineko probably due to her loneliness she felt from the hatred by humans, each Human-like has already been shown to have a trigger that made them that way. Wait, she gave the fruit life only to eat it. She's even worse than I thoughtI'm wondering if Raineko is the progenitor of the human-likes. Getting involved with her increases your chances of turning - which is why human protocol is to leave them alone and minimize interaction.
Fruit gains sentience and the ability to hold a conversation because she spoke to it. That girl on the train became one of them simply because Raineko said so while talking to her.
