Alcohol patches are something the people in charge of preventing alcoholism tried to implement to indicate to people beforehand how well the body of an individual would resist to it. Asian people are prone to not resisting it at all, the mangas are not exaggerating it.

I didn't find anything concrete on it, but since it is a medical test, I think it's not surprising that minors could take it to prevent alcoholism (since it would be very naive to assume they wouldn't be able to be alcoholic before age 20).

Honestly, it seems like a good idea, it will not work on some people sure, but if you lose all your means on just some patch you put on your arm that would be very convincing I think lol.

Edit: So I checked in more details how it works: It is not checking your resistance to alcohol really. It is checking alcohol intolerance. So our protagonist is like that. It is not like drinking alcohol, we're just checking if something bad happens when you put the patch (like red skin, itching... I'm not a doctor, so I will not pretend to be precise).
