i’m not a professional therapist, but in chapter 12–13, when he sees the doll, it clearly reminds him of his little brother. the moment it slips from naoki’s arms, he instinctively catches it, but as soon as he realizes he’s been “caught,” he goes back to acting erratic. i think this shows that the act of catching the doll comes from his guilt and the memories tied to his brother, even if his outward behavior seems chaotic. i dont know how to explain this, but i feel like his behaviors are tied to pain, guilt and memory! he sees the doll, it’s not just an object; it triggers something deep, like a connection to his little brother or a moment from his past that still haunts him. that’s why, even if he loses control and slips into erratic behavior, he still almost instinctively catches the doll when it falls. it’s like a reflex tied to guilt or something he feels responsible for! it’s almost like he’s afraid to be seen as vulnerable, or afraid to face what that memory really means. the mix of guilt, memory, and fear of being understood or exposed. does anyone see where im coming from or where im going at? i can see and understand where naoki is coming from, but we're gonna have to find out soon!! :)

Unfortunately this is the last we ever see of Sanou so we'll never know what's up with him. I think he's kept ambiguous to screw with Naoki's head and teach that he can't psychoanalyze his way out of everything
