So, this is pretty easy to explain. If you look at previous chapters, it looks like Ao-kun has dyed hair. Now, if he has spent time in a coma, and his hair has grown longer than usual, that hair would be his natural color (in this case, black), so if he cut his hair without time to dye it, it would be, again, his natural color.

There's been loads of foreshadowing in this manga, and they haven't expanded on it so far. It will probably happen soon tho, with how the manga is currently progressing.

A possibility is that there's way more depth in Ao's character than we think, and his involvement in all of this. After all, his sister was a target. There's also the fact he wasn't born in Japan, which probably connects somewhere, otherwise it wouldn't have been mentioned.

There's also this weird reaction when coming face to face with Peter infront of his house.

Kind of a weird response given that he just came out of his house. Did he know Peter before this point? I think peter did end up helping Ao, as he always said he is happy when Nezumi is happy, which she would be with Ao:

Also why is he holding a head here? Whose is it? Is this what he used to fake Ao's death? Pretty sure he's holding a wig here. He has hair in the previous panel, and no hair when he's holding it. Let's not forget the doctor lived right next to them, and he called Ao pants-kun, which means he knew him too:

There's a lot of other unexplainable pages, and they will probably tie all of this together at some point so it will make sense.

I don't think Peter is loyal to the organ trafficking group. He said in an older chapter that didn't understand why they did what they did, and he didn't know the reason he was born, until he saw Nezumi. I'm guessing he teamed up with Ao instead. How exactly they fooled them is still a mystery to me

Super Solid Take on this.
1. Im really curious to see how Ao-kun;s back story will come into play.
2.Ive re-read the whole thing since the head arc, and I noticed that Ao kind of just shows up and is very driven in his pursuit of Nezumi. Feels like his Sister back story will play into this.
3 Additionally When the Dr. was mentioning his original plan of Nezumi killing Ao to make the perfect killer but the Koreans messing up the plan by just showing the head; i wonder how this comes in.
4. Also the Pants-kun thing is weird! Ao did seem to know who Peter was.
5.I wonder who organized the head swap?
5. Finally I dont think its Peter with Ao's face. Ao was always shown fairly tall, the height of Ao in this seems right.

Ao-kun met Peter before some chapters ago but the author never revealed what happened after that
