Translating “オカマ”* as “Twink” is fucking insane—just look at the guy!—but that is what they did.
*“okama” does not have a great English translation. It refers to this kind of effeminate gay guy, often derogatorily, so the closest English word is probably “fag”, but that doesn't help with the pun in this case, so I would've just left it as “Okamassage”.
