If we're using Cthulhu Mythos powerscaling here, Death is just another alien creature here vs. the entity in the pocket dimension being on the level of a Great Old One (they're not even technically god-like in power according to the Mythos)

"The Great King of Terror" was summoned by one twat that was obsessed with avoiding it. Wasn't even a big cult or some crazy effort, what could a single dude even do by himself while holed in his house? If this didn't happen before it's just luck, meaning humanity has always being at the brink of extinction, one weirdo away from the end of time.

Just in case people forgot.
Gerai is from chapter 4, it is an unknown God (presumably an alien) in the form of an angler fish, who overtook a mountain village (Mugai) and devoured all surrounding spectres and even the original mountain god.
And the woman is a former Miko of the original god, but now is the caretaker of the spectre Village.

I think in this case, death is more like an outer god, where it literally cannot exist in this reality without using a much weaker avatar that was still incomplete when it got trapped, while Gerai just wants to eat instead of being a universal foundation of reality, having no need for an avatar to manifest at all.

It's mostly a case of Gerai having a power that's super convenient in this case in saving people, not that it is the only god capable of beating death, would be interesting to see how powerful the avatar of death is when complete though.
