I don't think either his mom or sister have pierced ears, he's an introverted shut-in, and he had already psyched himself out before he went into the building. It's not that crazy for him to see a charm with a loop and conclude "earring" before bolting out of the store if he's not used to seeing people actually put them in or seeing them sitting around his home.

It's certainly not the wildest reach the series has made.
