The MC is shocked because his sister said something nasty about him not having kids while he thinks she knew she was pregnant - basically lording it over him despite the fact he really wants a child. The daughter cares because they've manipulated a rift between the siblings by telling the sister that Hiroto wasn't as hurt by her comments as he was, so she would announce her pregnancy and upset him. The siblings never actually talked after their fight so nothing was really resolved between them directly. The sister's child is from the affair she had.

oh i thought the sister was happy because she was having incest feelings for her brother but knew it was wrong so had other children with other men but thought that also just pushed him away and wwas resentful that she didnt have children from him so lashed out with her previous words. The was reassured that he would still love them as his own, so was happy over that. Like, she was relieved that he would treat her children with other men like he would as if it were his, and she was being crazy like that.
