I think he briefly appeared previously in family photos and/or flashbacks - He has dark hair like Mihari and the ahoge was his contribution to Mahiro's genes. Though Mahiro's ahoge has grown much larger as he got older, and even more so when he got lolified - probably just because his (her) hair is longer overall.

Mahiro's gender identity is kinda ambiguous. We aren't sure that Mahiro wants to switch back still or not.

However some examples in other genderbender series like Ranma will still identify as male so uh yeah I'll use male pronouns if he identifies as such.

There are also cases like Tachibana from Fabiniku, who openly identified as female, but still uses male first person pronouns. I don't know what pronouns would be correct in that case.

You can see Mahiro's not-quite-a-PS4-controller several times in Ch87 as well. It's had a few other minor appearances from what I can remember, like the start of Ch103.
