There are two modes to the first name basis there:
One: you two are extremely close.
Example: Tsuyu Asui from My Hero Academia. She wants to bypass the formalities and become really good friends. This is why a shy and polite Deku struggles with it; without the social conventions they might appear to be a couple.
Two: extreme disrespect
What Kenshin Himura did to Makoto Shishio at the hot springs. No honorifics means great disrespect, and Kenshin is openly admitting as much. Shishio even calls him out on it.

When the slightest mention of japan in a postive light leads to comments akin to "stfu weeb lol" you really do not have any grounds to claim 'overreaction'.

I never really understood this Japanese cultural thing in saying one's first name. I mean I come from an asian country myself, but probably because of colonisation and western influence, the usage of first names became a norm for us if that ever really mattered to begin with? It just doesn't make much sense to me unless you spend the entirety of your life being called by your last name thus making it strange when you hear your given name. But even then, shouldn't you feel empowered rather than embarrassed when you hear your name spoken when its such a sparse occurence?

Anyway, I guess I'll just never understand the intimacy of calling someone by their first name and it making them flushed.

It's basically like that, let's say your name is Alexander, everybody calls you Alex but only the closest being to you calls you Xander
Now when your friend realized your girl classmate or whatever calls you Xander, they'll know something is up with you both, whether it's with or without a romance, they just know you're close enough to using a nickname like that. You'll also feel somehow weird (in a good or bad way, depends) when someone other than your family use it for the first time
For Japanese, it's much more than that but the basic premise is more or less the same

Mine also have, not first name, something can lead you in familiar situation. It simple implied you are very close to the said person or in relationship What so hard to understand ?

That depend on the person, not culture. Like some feel embarrassed seeing a kiss scene on TV. Some telling dirty joke at school as something to proud at
