What I think is that Kouichi's trigger is more or less a deep desire to fit in with them (something like Jealousy), but there's hesitation as well because he has been living a boy for a long time now, and the transformation happened late. It's jarring for him.
What makes it worse is that he doesn't have that much time before one of them becomes his permanent figure. If it that happened earlier or the fix happens a lot later he would have a lot of time to decide. But then again, he has been living male his entire life until now so it's more natural for him to choose that.
It doesn't help that the others won't let him be. My boy- er girl doesn't have peace when it happens lol.

Poor MC. Unlike the others, he doesn't want to be a girl and he hasn't had years getting used to it like they have. He might get stuck and go through puberty all over again, this time as a girl.

Looking at the bright side, when (I guess that will happened) MC will get stuck as a girl she will have much more time than the other since she'll be in the body of a 10 year old... not much and it probably will be frustrating at first but it's a few more years of living and experiencing a girl body so maybe she'll settle better thanks to that?

One can only sympathize with kou, being pressured into a gender identity hes clearly uncomfortable with by his family and peers... wait that situation sounds familiar...
Anyways yeah obviously that wasnt gonna work cause how else are we gonna derive comedy from the protagonists suffering for the rest of the manga! Im more suprsied that the others didnt mess him up during the one week and we just skipped over it, it kinda seems rushed.
