Well there is a real life concept called the Spontaneous human combustion, were people have been claimed to burn alive without any external trigger. Though reported case are known, there have been no scientific evidence for it yet. Some even claim that it could be related to some medical issue.

As for this series, the reason they didn't think about it being a Curse because they didn't consider someone applying it on the Lord's Sister. Most of the people probably considered it as some form of disease or affliction that she might have suddenly contacted.
I mean, black scars can also appear when a person is subjected to Frostbite.
