The dark spot is still noticeable in the first panel of the next page, and what looks like it could be a bruise on her cheek. In that one it's actually kinda freaky how half-open her eyes appear, like she woke up or died as he was moving her head to his lap. Not to mention the creepy and/or freaked-out smile he has at the end.

…the more I look at it, the more I get into a state of mind where the "Zzz... zzz..." is the only thing that keeps me from believing the motion lines in those panels are actually death spasms.

Hits her head when she falls asleep I'd guess. Like they said earlier in the chapter, they can't wake up after they fall asleep because they've already gone to another loop.

I wouldn't say creepy, he gives her a lap pillow because he cares about her and view her more in a platonic way. If it was up to her they'd have sex like bunnies already, but he cares too much about her to exploit her like that.
If this goes like groundhog day, my guess is that she's looping because she doesn't really love or care about anyone, and she won't stop looping until she really cares about him. Or something like that. And maybe we'll have some drama where they fight and he ends up leaving the loop or something if I had to guess.

If it helps, I was saying his facial expression was a "creepy and/or freaked out smile" -- not his behavior or character. The kind of smile you'd expect on someone of good ethical character who feels helpless to change their situation, and the only bright spot they see is "well, at least when she wakes up she won't have a concussion / be dead".
