Most Favourite?
Definitely Steins;GATE, Higurashi No Naku 1&2 is a close second
Least favourite is a bit harder... Maybe Studio Deen's Fate//Stay Night Fate route adaptation, which very much butchered it? Something that I found particularly enraging after reading the VN. Otherwise, that's a bit harder to answer, due to me not really watching anime most of the time.

My personal Peak is Keyman - The hand of judgement, Spirit Circle, Hyouge Mono, A Beast Cries in the Wilderness and many others, though the named ones are certainly standouts here
Least Favourite... Nisekoi.
Definetely Nisekoi.
While I have a fair share of others I dislike, I hate that one with a passion.
