idk if I'm reading too into it, but maybe he really did forget he called her an adult because of him subconsciously burying his feelings for her. He thinks he's too old for her so he just treats her like a kid in order to not fall for her and hurt her and himself.
However, when that worker wanted to insult her as a kid, he automatically went to defend her because that's what he really believes. Honestly, the whole problem can be seen at the very beginning where he was relieved that she "didn't want to marry him" because he's too old for her. He never said he didn't like her, he just thought himself unqualified since he's too old.
However, she's too tsundere so it'll be hard to break that misunderstanding.
