I once read the manga where this girl develop another personality whenever she got too excited in her childhood to keep her from heart attack, anyone recall?

Nope she wasn't the MC, but yes, I think that's the one, the MC is a normal student who he thought he played with 2 girls in his childhood

Turns out it was one girl all along having a second personality to play with him separately

He realized it when he confessed to her in Highschool, she got so excited that her second personality comes out

I didn't know you meant literal personality shift until the second post. I was thinking of Ruka from Rent-A-Girlfriend (shitty manga don't read it). She played down her real self because getting too excited effected her heart.
