Based on what I've heard from a few people over the years, it can be exceedingly unexpected for the person themselves, especially if they don't have a history of various ailments. If you have never seen one in person, or any other similar conditions, it's possible it will take a while to react when someone starts to show the signs of one. Japanese, in particular, are rather healthy people, aren't they? No obesity problems to speak of and plenty of fish on the menu. So, it's even less likely someone still in the workforce and not involved in the practical medical field would see cases much with their own eyes. The only factor that will certainly change all that is the aging population in Japan.

As someone who's had a person nearby had a stroke and collapse? Far more believable than you'd think.

Took a look at the current raws

Kaoru’s dad is hospitalized but seems to be okay. Fortunately since the other guy was there, Kaoru’s dad got the help he needed in time. What worried them if it had happened while he was traveling by motorcycle and no one was around, it would have ended up really badly. Nanami imagined possible fatal accident with broken helmet.

Nanami spent the night at Kaoru’s place to be emotional support. And before that she skipped school with him to be there for him when they went to the hospital.

I've had a minor stroke and even that is fucking frightening as all shit.

I'm really appreciating this last arc being about Kaoru's problems. Nanami and family are gonne be the crutch this time around and I'm going to ugly cry.

An old work collegue of mine had a day off and drove with his wife and their motorcycles a day tour. When they arrived at a café, he parked his bike, took of his helmet and started to walk to the café. Suddenly he had a headache and fell over. It was a stroke.
He was lucky, there was a doctor nearby. He couldn't work for about 6 months and after than for about 50%.

Well, I was initially thinking he was run over by a truck or something, so at least he has a chance to survive this. Maybe the power of Kaori can save his life...
