Really? No one noticed that guy has a stroke? With that weird expression on his face? C'mon, I just can't believe it.
Edit: And he got on a motorbike to work, while having stroke. After which he almost immediately collapsed. I mean, sometimes I think it's better, when authors stay with simple "mystery illness".
Based on what I've heard from a few people over the years, it can be exceedingly unexpected for the person themselves, especially if they don't have a history of various ailments. If you have never seen one in person, or any other similar conditions, it's possible it will take a while to react when someone starts to show the signs of one. Japanese, in particular, are rather healthy people, aren't they? No obesity problems to speak of and plenty of fish on the menu. So, it's even less likely someone still in the workforce and not involved in the practical medical field would see cases much with their own eyes. The only factor that will certainly change all that is the aging population in Japan.
