This is an alternate timeline. The diverging point was when the Nidaime club shoved Madarame and Saki into the clubroom. Madarame actually confessing leads Nidaime happening as normal and him dating Susanna. Madarame not confessing leads to Spotted Flower.

Yeah they kept it vague for a long while but there are hints saying this is different from Genshiken (this manga started before and then serialized alongside Nidaime). But the current batch of updates showed the flashback where he ran away while in Nidaime he got shot down. Saki also said that him not confessing lead to this very future. It's all very new revelations.
