I mean, sure it doesn't have to involve sexual intercourse or oral relationships. The way I see it, when someone forcefully interferes into such relationships like that mega pervert, it almost feels like a sense of betrayal, which is something that aligns with the whole theme of NTR.
NTR often involves infidelity and most importantly, betrayal, and I believe that characters don't necessarily have to be in a romantic relationship for each other to feel that way.
The way that the balls seem to transform into more girls is almost like procreation, albeit involuntarily, which doesn't help very much, at least that's how I interpreted it. The alien-like nature of the girls also further supports this idea, since I'm not exactly sure if they are able to reproduce like normal humans.
Imagine an anime high school male protagonist who always had feelings for his female childhood friend from the start, and she also has feelings for him. But then, he suddenly finds her being kissed behind the school by his bully. There's that sense of betrayal and cheating, even if they aren't in an official relationship. The bully is doing this to assert his power and dominance because kissing is often intimate in most societies, and it is generally seen as something only done if both parties consented beforehand, and if they are fully comfortable with what they're doing. In this case, even if she was manipulated into kissing the bully, I doubt she has any real honest feelings toward him. It's like even if you force someone to do your bidding, they'll never truly love you.
You could argue that it is the boy's fault for not confessing or letting the girl know about his feelings, which leads to the situation being taken advantage of by the bully. But I'm sure most high schoolers are shy about confessing to their crush and not until later in their older years or maybe not at all. This scenario is also an excellent lesson on itself but that's for another time.
Forcefully kissing is like a breach of trust, and it can make some people uncomfortable. Imagine being kissed out of nowhere, by someone you don't even love, that would send you into a sort of surprise and shock.
I understand if you and I have different opinions on kissing and lip exchange, as well as what infidelity or cheating means to you, and it's totally fine to discuss about it. I like talking about these sort of things.
