I've known a fair number of such couples, and not in Japan, in France. It worked pretty well for them. I don't understand it much myself as indeed I would expect this difference to be too much an hindrance, but the fact is that it can work depending on what both lovers expect from their relationship. I guess that as long as both are major, there's little to say.

It comes up from time to time. The phone thing, the tayama talk, it's just they spend very little actual time with each other for it to come up often. Not to say their time isn't meaningful but they don't talk about music or movies or pop culture or things where the age difference would show up. They mostly talk about things that are relatable at all ages.
