wait, I see people in the comments mentioning the goddess? Who is this goddess you speak of? Did I miss something while reading this manga? Also, I don't care much for spoilers so yeah.

I don't get it why suddenly and OP turtle appear, where the 3 idiot knows about the turtle, why the bird queen said there is 2 option, for 'the life or the world'? what does it means? I feel like after the 'bitch' and 'trash' get punished, the quality take a nose dive.

@PoH this is a spoiler from the WN, so things may be a bit different from the LN, but
The turtle is the equivalent of a final solution. If it is able to kill enough people, then it can effectively stop the timer on the hourglasses, ie, stop the waves from occurring.
The 3 idiots knew about this because the games they played had a similar event to this, so they asked about the countries and saw one named Reiki. They connected the dots.
Fitoria is saying that because you have to choose between letting the Spirit Turtle do its purpose, ending many lives in return for stopping the waves, or kill it and risk the waves of calamity destroying their world by attrition. The unfortunate reality about the waves is that this isn't the first time these have happened, nor are these the first heroes summoned. Anytime the waves happened, maps had to be redrawn after countries were destroyed as a result of them, despite the efforts of the heroes.
In effect, Fitoria is giving up hope that the heroes will be able to win against this threat, so she is leaving the choice to Naofumi... let the Spirit turtle kill for the sake of the world itself, or kill the Turtle to make sure those lives aren't extinguished. As it is, she chose to let the Spirit Turtle accomplish its purpose, guaranteeing the world doesn't have to deal with the waves again.

The entire conflict is happening because a goddess named Medea Pideth Machina wants more power. Essentially, when she destroys a world, she can collect the energy from it and empower herself. She actually has an entry on TVTropes. Anyway, because she gets bored of destroying the worlds herself, she makes it a game by causing the waves of calamity by destabilizing the world itself. For this instance... the world originally didn't have demi-humans. They became a thing after she took one dimension with demi-humans and slammed it together with another to form this one. For how much of a bitch she is... for context, Bitch princess is a shard of her, ie, a small bit of her. Imagine Bitch x100, and you might get her. She is also why the world that Glass, L'arc, and Therese is fucked up too... but she decided to not do waves. Instead, she infected most of the chosen heroes with insanity to turn them into blood knights who lead kingdoms. Glass, L'arc, and Therese, ironically, weren't even the original heroes, if I recall correctly. They were chosen after the previous ones died and got lucky that Medea stopped looking so closely when they were chosen, so they didn't get infected with bloodlust.
As for being confused about it, don't be, she doesn't even get mentioned until like chapter 300 in the Web Novel.

I still don't get why the waves keeps happening. Like there is a lot of holes in the world making. Why there is multiple hourglasses? What happen to other country that don't have heroes? If the heroes gets teleported to the wave location, why they don't get teleported when a wave happen in other location? Hell we don't even knows why the wave is happening in the first place.
Just for this turtle alone. If Fitoria knows it, that mean this already happen at least once, so the Heroes managed to kill it? because if they don't the waves already stopped. Why the turtle is even be able to stop the wave?
At first I love this manga because of the not so goody two shoes MC, but after a while it start to revert back to 'I'm your really generic hero that will save everyone.'

I'll answer those questions as best I can, but I may not be able to answer them sufficiently. This is taking from my memories of the Web Novel from over 2 years ago, so things may be off, or even because the LN is different than it now. Remember, these are spoilers, so if you are fine with that.
There are multiple hourglasses so each of the countries can tell when the wave is near. There isn't really more reason than that but makes sense so the countries can prep their soldiers and subjects.
Originally, the major countries were supposed to summon one hero. We aren't sure which would get which hero, but the demi-human country was pretty much guaranteed to get the Shield hero, well, up until the King fucked that plan up by summoning all four of them while the Queen was away negotiating everything. He pretty much did it so he could get the shield hero to fuck with him while denying the demi-human country of help, hoping they would get weakened by the waves. That was also why it took so long for the Queen to appear. She was making sure her country wouldn't get fucked by all the other ones for this act of stupidity.
The wave is technically happening all over the world, but the heroes get sent to where the invading monsters are most concentrated. Though, I could be wrong. It may also be the scenario where they get sent based on which hourglass they are closest to. This is the one where I just don't remember well.
And the waves are happening because the world itself is unstable. TL:DR, the world was originally 2; one with only humans and another with only demi-humans. They got smashed together to make one world, and the waves are a brute-force solution to restore order by destroying said world.
The turtle is basically able to collect all the magical energy of the beings it has killed and use itself as a dimensional stabilizer. If it is able to kill enough, it can effectively keep the waves from happening again. Technically, it isn't infinite, but it is close enough. The last heroes sealed it because they didn't want to use such a strategy if there are other ones still on the table that are possible, but have it available as an option if there is truly nothing left.
I would say it never truly reaches the point to what isekai is like now... but, it does go back to the overall theme of good vs. evil inevitably. This is the scenario where all the people in the world eventually band together because they have to choose between fighting each other and guarantee they die, or band together so they might live to see tomorrow... when they technically are free to kill each other again. Even if that happens, it means they have a future again.

It'd have been way better with the turtle ending than this. I guess they couldn't be able to milk it that way, huh?.
