Or maybe he's just going to tell her that he's hired somebody to infiltrate her office every day, until the day she retires, and tilt all the paintings.

Going to guess that the 2nd reason is actually something powerful.

1st reason is sorta harassment so what reasons do they have to listen to that.

2nd reason has to be something worthy for Sofue to bring to the professor meeting and not ignore.

So it could be a blessing to Katou but Noguchi views it as a curse

She would have to spend her entire life changing the department, with him looking over her shoulder. Should she fail, she would know everything is pointless. She has to devote everything to change, sparing no energy elsewhere like making a family.
Noguchi knows she can change thing, so his line is instead of stopping her, to push to do everything in her goal.
In regard to the intern, he would say he supports that,and double down that Katou have to let other interns work in the future [so Katou can't just say it him] so interns mess up,it on her.
I'll support you,but let see if your path is truely clean. If it is sure. If it not,you will have a greater fall then him.

Kirishima tried so hard that i thought Sofue would kick him. Good for him that he likes people to be servile.

It was obvious Noguchi would endorse Katou as that's the main way to make both fail or at least struggle like he said in the previous chapter, but the reasons and the plan behind will be funny for sure.

I think Noguchi will end up redeeming himself by choosing Katou as his candidate. The manga is almost over and after his surgery, there must be some sort of conclusion to his character arc.

I must say that Noguchi had one of the best character developments in this manga.

I think Katou is marginally better just on the basis of adapting to the future. This is a double-edged sword. On one hand Katou is using people as guinea pigs, experimental operations that have a low chance at success. On the other hand, much of what Katou has done - has been successful and saved lives as a result. Noguchi himself wouldn't have been alive if Kirishima had been in charge. She's willing to have thrown away her 'humanity', to do such experimentation and get results. She's willing to allow an intern the opportunity to grow unlike other people. Kirishima despite claiming to want everyone to progress and increase the overall ability of 'average' doctors, threw Kihara away on multiple occasions because he wasn't capable enough. Ironically he became like everyone else, and a large portion of that stemmed from trying to save face for the election. You could argue that he would continue to play into that facade even after he became the Head, much like Noguchi trying instead to save face continually without implementing any crazy changes. The same could not be said of Katou.

I think that's why he ultimately chose Katou.

I reaaaaaaally don't buy Sofue saying that he's gonna vote for Gunji. I bet he's voting for Akira in the real voting just to shit on Gunji. Even I feel rage from reading the unrespectful shits Gunji said to him.

"Now there's no way I can lose!" Bitch, there's still Kitou's faction. If Ryutaro manages to change Kitou's mind about pulling him to the ER (like, say, pointing out the injuries in his arms and hands from catching Shingo), then you are dead. Your slimy personality doesn't mesh well with Kitou's after all.

God, I can't imagine how it feels like to be Akira in that room. Takeo is still sending me chills when he does this kind of shit. Regardless tho, I think the atmosphere of that room will end up bright, like for example from Takeo saying that he always thinks of Akira as his daughter-in-law or something like that.
