I think it's half and half here if you apply our logic to their world.
If he was ugly then he could only pull desperate/older mid to bottom tier girls.
Perhaps a surprising amount of mid tier women would settle for him in a brothel over time if he's cheap enough and they don't have other options readily available.

I guess it's true that sometimes high tier dudes in our world will sleep with a woman waaaaaaaay below their rank on a lark (" I've never done X with fat chick!"), with beer goggles, or if they have a pathological urge though.

The Male MC seems to be at least average so he's able to pull some top tier girls looking for a "practice" dude (or just horny enough for a one time fling) along with average to above average chicks like Kawashima who are willing to pay and are horny/crazy enough to try making him go exclusive with them.

The women in the reversed world are less adverse to openly and knowingly sharing compared to men in the normal world.
I believe there was a chapter that showed that reverse world women still retain the "men can impregnate and care for many women/seeking the best male specimen makes it worth sharing" type of logic because men in that world still fight the wars and have a tendency to be the bread winners.
