Even if Ishiguro didn’t specifically use say morning sickness as a symptom to foreshadow pregnancy, he likely wouldn’t use a symptom that isn’t super common in pregnancies that is simultaneously also specifically universal to periods (also combined with other symptoms of periods, i.e. cramps).
Also Kiruko’s sense of smell isn’t heightened, a bed that Maru’s been sleeping in continuously for the past few days is going to smell like him even to people without a heightened sense of smell.
