Technically it is the mother fault for not taking care of herself well enough while pregnant.
Though, since noble, I suppose it is the family doctor's fault for not doing his job of keeping her healthy or made some error during procedure.

From what I know, it isn't much but something, only Amniotic fluid embolism is the only cause of death not related with lack of health in the mother and can be caused by luck or medical error.

But what happened to the first wife ? The brother's mother ?
Did the father just divorce her and then remarried to leila's mother ?
Why is the brother so sad leila's mother died ? it isn't hers and he was already "old" when it happened, they didn't bond much. Manga skipping through important exposition I feel.
