She has to have existed before he went to the other world. She's been in her current reality for years. At the very least, more than the few hours that the MC noticed he had been gone for. The only alternative to her existing in the same reality with an MC that hadn't been isekai-ed, is that the MC didn't return to his own world, but an alternate world/parallel world.
