I'm gonna be that idiot that takes your joke seriously and explain it: If I remember properly, her current constant facial expression is precisely colateral psychological damage of living in an environment so stratified it doesn't find value in you if you are a mere R.

It's a defense mechanism after so much rejection. Instead of, let's say... training aimlessly like Urus does. She also happens to be the embodiment of some kind of nihilism in the series but she didn't use to be like that... Putting a tough front for so long eventually turned herself into a horrible and manipulative person in the process.

In fact, every single character on the series (Even the stronger ones) have their own psychological traumas... And the system procures to perpetuate them.

Honestly tho, it could have been worse than it is. Most of the psychological damage comes only from the characters' sense of self-worth. If you truly wanted it to be hell, you would also ensure your prisioners.

1- Lacked any privacy what so ever.
2- Making it so the means of subsistence would only be given to those on top. Making the rest fight for leftovers.

The headmaster might be a piece of crap, but she knows that all the money thrown in the gacha for the high ranking heroes is enough to fill everyone's belly. If this was a drama and not a comedy, things might be a little bit more harsh.
