The only things I can think of are her just eating something really bad and not being able to stomach it, or Bulimia, which can I think can feel pretty horrific if someone catches you in the act. It could just be that she's embarrassed about it being in the classroom instead of the bathroom, which is reasonable, but not really something to warrant a whole """manga""" about.
