When in a panic the driver could easily just keep stomping the brake. In the accelerator mistaken for brake accidents some people have stress fractures in their leg because their pushing the accelerator so hard trying to stop.
If your brakes fail you should pull the handbrake if you even remember where it is, turn off the engine, and if worse comes to worst slam the transmission into park (guaranteed to break something though). Turning off the engine while in drive should really slow you down.
I haven't tried it but I think it'll work...

There's absolutely never been taught how to act in situations like that in a standard drivers learning process.
Something like learning how to act even when loosing control isn't on any tests, it usually takes about few months at most, skipping bad weather or unexpected conditions is mandatory.
Handbrake won't immediately stop a vehicle, neither will put it in park or turning off the engine as the momentum will still be in the effect, this isn't a bicycle.
As the speedrun this story is, eventually the truth would unfold and Ayana would either snap back to her delusional world or remember it all while carrying the guilt, it was still her fault but that was an accident nobody would predict.
Kana jealousy is at it's peak, from supportive best friend to a homewrecker, knew she would abuse Ayana psychotic breakdown eventually to get closer with Riku, sadly it's unrequited love once again.
