Having read the novel also, I'd say Raphael never left Cayena to die as he couldn't know her situation. He didn't get the letter where Cayena was asking for help, because her husband discovered the letter before it was sent.

But yes, Rezef might have left her to be tortured. However it was never revealed, whether even Rezef actually recieved or read the many letters from her sister. There's a slight possibility that his aide destroyed the letters. Though I'm more inclined to believe Rezef simply didn't care

To those wondering:

This is Cayena's third life.

First she was the og stupid Cayena

Second was modern time girl, who died to a stalker and read the book about the first life

And now the current where she is Cayena again and remembers both previous lives.

Raphael never rescued her in the first timeline because the only letter she sent to him was intercepted by her psycho husband, whom Rezef sold her off to, after he was done using her to get the crown.
Raphael didn't like her in the first timeline, but he is a just character, and is too honorable to abandon someone in need, no matter how he feels about them.
Keep in mind that he didn't really do anything bad to her in the first timeline, despite her constant, relentless harassment, and the only reason he stopped tolerating her was when she poisoned Olivia.
He also forgave her and gave her a chance quite quickly in the current timeline, him not helping is too out of character to be feasible.
