How do you know he/she has something between his/her legs?

Son had short hair, got pads + wig.
Dead person picture was woman with long hair.

That doesn't mean anything.

You can easily and quickly cut long hair short. Growing out your hair takes time, wearing a wig doesn't.
It's also not unusual for women in Japan to wear pads.

Dad is el nutso.
Son still thirsty af and unhinged.

We don't know that.
Maybe dad is crazy and thinks his son is his wife. Maybe his wife is crazy and thinks she's the son.

However, there are a number of clues that mom is the crazy one.

1. Chapter 2: Mom's clothes fit the son perfectly.
2. Chapter 8: There's a picture of mom and dad with a baby, next to a picture of mom (first seen in chapter 5). The picture of mom is probably not a funeral picture, as it's highly unusual to put a funeral picture next to pictures of living people. Also note that the picture of the baby is the first (and so far) only picture of the son we've seen.
3. Chapter 9: There's a crib, and it's empty. There is no mention of the son having any younger siblings.

These clues suggest that the son died when he was still a baby, and that the mother really did pretend to be the son.

None of those clues are decisive evidence that the mother's crazy, but there's also no evidence to support that the father's crazy.
