What separates a gravure idol from just a gravure model tho, or even a JAV actress? is it just numbers or the company promoting her products?

gravure idols - just regular idols. Though that said they get sexualised by the online fanbase just as much as everyone else, and it's not like they didn't in the past have some extremely shady stuff with girls of that age (eg. Ai Kago, early S/mileage). It's pretty fascinating though - now more and more idols of that agency are opting not to do swimsuit photos and be overtly sexualised (after a few of the influential senior members refused) and it's lead to a lot of discussion online about the differing attitudes of new generations and such, not to mention a lot of anger that they, as fans and supporters, are being "denied".
And the difference between "idol" and regular model? I'd say it's that an idol is a brand unto themselves - so it's not just about them appearing in a thing, it's about them being in that but also cultivating and appealing to a fan base, creating a sellable character, diversifying their career with variety show spots, usually being part of an agency promoting them, having fan events, etc. It's really the parasocial element coming into play.
