Ch.13 was pretty cut and dry, since it led right into Chitose's dialog right after. Not sure how it's confusing there.

The thing people are speculating about is a far more recent flashback (Ch.92) in the burnt-out ruins of a city with a wounded little girl and an older girl or woman facing each other, and they made it ambiguous which was Tokiko and which was Tsukasa.
