@thegentelman wtf!? just... why?
@thegentelman its just perfect.
In all seriousness, it's actually a deerskin hair ornament. One of the Q&A sessions at the end of each volume actually explicitly said as much. The series wiki calls it a barrette, specifically, which seems about right, to be honest, since many barrettes can look like that.

why are people commenting as if they never done it before?
Probably because they actually haven't, but people assume they did after some of the kissing/hugging sessions up until this point in the manga. If they had done it, they probably wouldn't be so avoidant of actually taking a bath together at this time. Of course, they do end up using the private bath together, but they remain such adorably innocent derps the whole time, if the anime is anything to go by. It's still pretty obvious actually having sex is still a ways off even after that.

Seriously, though, if they had done the deed, they'd be a lot more comfortable around each other, I would think. But I'm really concerned her healing factor would make it too painful, and every single time, too, so I really think her heavily-implied immortality situation needs to be addressed first.

I need to look up Kusatsu and Yoshimune. This is apparently another one of those Kaguya-hime hints.
If the theory about Tsukasa being Kaguya-Hime is true, then I wonder why she's getting flustered over everything, like it's new to her. Surely she could've experienced some of those during her time wandering Japan centuries ago.
I believe it is due to her maiden's heart. 😇

She seems to be perpetually 16 years old. Her approach might be affected by that age limit?

Though Kaguya-sama Wants To Be Confessed To / Love is War has given me some more info about this Japanese cultural aspect, I don't know the full story. I do know she gave her suitors some impossible tasks to accomplish. But in this case, she is dating Nasa in her day-to-day relationship, though they're married in the legal realm. But that was the premise: "Marry me and I'll date you." So, my understanding is that they're simply dating from her perspective. 🤔
Pretty much what Fujiwara no Sai says, more or less. Reminder she's still physically a 16 year old. She's been stuck that way ever since she, presumably, drank the immortality elixir that was supposed to be burned atop Mt. Fuji at the end of the Tale of the Bamboo Cutter (Kaguya-hime story), but, based on my understanding up until this point in the manga, must have been instead given to Tsukasa during her 16th year of life. Most likely her father back then was one of those tasked with the elixir's destruction, and instead of following the imperial order he gave it to his daughter. Of course, this would mean the inevitable happened and her father was executed for defying imperial decree. I can only imagine the sadness Tsukasa has gone through.

Anyway... She's innocent because she's never been physical with anyone or fallen in love before, far as I'm aware. Nasa is her first love, despite over a thousand years wandering the Earth. She has lived a very solitary, sad life, most likely, because of her immortality (again, refer to the most probable loss of her father as a consequence of her immortality). When everyone you meet and grow fond of eventually dies and leaves you, you grow cold and distant to everyone. But, for whatever reason, Nasa caught her attention. She wanted to give this bold young fellow a chance to love her, and receive her love in return. There might be more to it than just his boldness of asking to date her despite his injured state, but that's a possible mystery for later.
