Very late reply, but it happens often in manga, and I don't like it, either. Usually, it refers to someone not in the panel, but some mangaka intentionally make it ambiguous. Sometimes, the translator doesn't know, either, sometimes because Japanese omits nouns, etc., so more than one character could be saying something.
