I'm still convinced that "spell card duels" are just games that people in Gensōkyō came up as maybe a staple banquet game. That actual incidents involved actual fights, and the "spell cards" are more like reviews of what happened. Like, they had a nice talk later and made the spell cards up.

I think even if formalized danmaku isn't how incidents are resolved, there still seems to be some rules in place governing the fights given how no one kills Reimu, even in the bad endings (well, Yachie almost does, but if/how people outside Gensokyo would know about such rules is another can of worms).

We may also simply translate losing all your lives in-game to mean the protagonist... well, runs away.
