It's not about prevention though, it's about proving that the disease is real, which seems to be a significant portion of the MC's troubles.
They established in the first few chapters that the disease is known to be real and that help exists for it, but that doesn't really make it any easier for the victims to prove they're victims of the disease. They'd still need to do a bunch of tests to prove whether or not the person is infected, and the whole time those tests are being ran, they'll be under suspicions of committing identity fraud among other charges such as breaking and entering, and theft. That's a lot of pressure on the person when a simple faulty test kit or false negative can leave them not only with no way to prove their identity, but also with a bunch of fines and potential jail time on top of their medical expenses because they can't be identified, they might even get deported because their citizenship cannot be proven. It also would not stop people from being weirded out by it, or from taking advantage of the insecurity.

You actually see people fall through the cracks all the time with the legal system like this in reality too, they fall victim to some crime and don't step forward because they fear the social repercussions when it gets out that they were a victim and the potential repercussions of the charges not going through. So they bury that fact and often end up perpetuating the crime, continuing to fall victim to it over and over until someone else reports it and the criminal gets taken in.

Actually, they never went to a hospital, I don't know where you're getting that from. Is it because the term "patient" is used by the narrator? The word is never directed specifically at her, it's a generalized statement for similarly afflicted persons.

In Japan, you cannot receive medical treatment without valid ID or someone that is able to vouch for your identity (legal guardian or police, usually) She got kicked out by her parents and none of her friends believe that she is the person on her ID. With her not matching what is on her ID, and no one willing to vouch for her identity, she would have to turn to the police to file a case and receive care but she hesitated to do so because of how strongly she was reject by everyone else, and she wouldn't be any less hesitant to go directly to the hospital with only the hope that they'd accept her story and take her in without having her arrested for attempted identity theft.

Comeuppance/karma is always nice. But damn, that's really sad that it is something transmissable like that. It already sucks enough to have the body forcibly change sex like that, but that makes it immensely difficult for one to find a lover even if they adapt to the new body, since there is the risk of causing the same thing to them. Which is just an even worse thing to add to the trauma this MC has already endured.
It's not just finding a partner, they would be treated as awfully as people with HIV once were and still are. Discrimination against people with TS disease would easily escalate to violence.

I hope the Author knows what he's doing because I don't appreciate making people with TS disease even more vulnerable to discrimination and violence just because he wanted to write revenge rape.
